    We are working with a fantastic healthcare organization based in Phoenix, AZ 85013, and we're currently seeking a Senior Patient Logistics Coordinator to join a great team. Your role will be crucial in coordinating patient care, handling critical thinking and problem-solving tasks, and ensuring effective team communication. This position offers a contract to permanent employment opportunity, where you'll have the opportunity to enhance personal and detail-oriented development while supporting care innovation.


    Available shifts: (12-hour shift, 3x a week - looking for availability 7 days a week)

    1) 7am-7:30pm

    2) nightshift 7pm-7:30am


    Responsibilities:


    • Act as a lead resource for the team, offering expertise on various applications and technology used in supporting patient care.
    • Monitor and escalate issues regarding delays in patient care transition management to ensure a quick, effective response.
    • Independently progress patients through the care continuum per established protocols.
    • Facilitate seamless communication with referring and receiving facility staff for timely placement of patients.
    • Accurately document all activities to promote clear communication with care team members.
    • Develop creative and innovative approaches to accomplish duties and encourage such thinking in others.
    • Complete all required reports accurately and punctually.
    • Implement and maintain an effective training program for new team members.
    • Maintain the functionality and perform weekly updates of Command Center Displays.


    • 2+ years of recent experience working in a healthcare setting
    • Must have the ability to coordinate effectively with a variety of customers including providers, hospital and office staff, health plans, medical groups, internal departments, community resources, and peers
    • Ability to work independently and apply critical thinking skills toward making appropriate decisions, as well as in a team environment
    • Exceptional communication skills, both verbal and written
    • Proven experience in delivering high-quality customer service
    • Familiarity with the healthcare system and its operational procedures
    • Ability to manage patient access effectively
    • Experience in coordinating patient appointments
    • Proficiency in patient scheduling, ensuring smooth operations.
